FORT GARRY PHARMACYCUSTOMER PRIVACY POLICY 1. We at Fort Garry Pharmacy respect and value the privacy of all of our customers. We will comply with the laws of Canada and Manitoba that apply to us in connection with protecting your privacy. These include the customer confidentiality requirements in the Code of Ethics of the Manitoba Pharmaceutical Association. 2. By placing an order with us, you thereby agree that we may collect, use, retain or disclose information about you in accordance with this privacy policy. 3. When used in this privacy policy, “information about you” means information in a form that can be linked to you. We may analyze or release data (e,g, publish sales statistics) in a way that does not identify any particular individual. 4. You may deal with us (e.g. place an order, provide information about you) personally, or through an authorized agent or representative. Either way, our rights and obligations under this privacy policy are the same. (Another person’s authority to deal with us on your behalf can arise from your agreement, or by operation of law). 5. We may collect, use, retain or disclose about you only for one of these legitimate purposes: • to process, and to deliver in Manitoba (to you or your authorized agent or representative), your initial and subsequent orders in a safe, ethical and efficient manner; • to carry out the ordinary commercial operations associated with a retail business, such as obtaining payment for your order and maintaining proper accounting records; • to comply with the requirements of the Canadian and provincial laws that apply to us in Manitoba; • to be able to respond to any claims or complaints against us; • to address an imminent threat to the life or health of you or another person. 6. When you place an order: • We may collect the information you provide to us; • We may seek additional information needed to process your order from your prescribing doctor or your health plan. 7. Within our company we will only share information about you among officers and employees to the extent that is reasonably necessary to accomplish the legitimate purposes set out in point 5. 8. When third parties, like our lawyers or accountants, provide a service to us, we may only provide them with such information about you that they strictly need to perform that service. The same principle applies when we deal with a third party providing a service for you, such as a courier or your credit card company. We will only deal with third parties whom we reasonably trust to respect your privacy, either because of the regulatory context in which they operate or because we have obtained contractual assurances from them to do so. 9. We do not sell information about you to persons outside of our company. The only exception is this: if we are involved in a corporate reorganization, or if we sell or lease all or part of our pharmacy business, any new owner or operator will have the same rights to deal with information about you as we do. We will take reasonable steps to ensure that any company that takes over from us, or any new owner or operator, has a privacy policy that is essentially as good as ours with respect to the protection of information about you. 10.When disclosure of information about you to a third party is required for one of these purposes: • to comply with the laws of Manitoba or Canada; • to be able to defend ourselves against current or possible claims or complaints; • to avert an imminent threat to your life and health or that of another person we will limit the disclosure to that which is strictly necessary. 11.We may, from time to time, send you promotional information about the products and services we provide. We will, however, honour any request you make at any time that we not send you such information. 12. We will take reasonable care to protect the security of information about you. 13. We will ordinarily remove information about you from our files at your request. We may retain it only if we have a reasonable need to do so for a legitimate purpose set out in point 5. We will, from time to time, review our databases with a view to deleting information about customers that is no longer required. 14. You have the right to know what information we have about you. If you find any mistakes, let us know, and we will correct them. You may contact our privacy compliance officer at 1- 866 - 887-0688. 15.
